Why a ankle MRI?

The ankle is a small joint with a lot going on — three bones, multiple ligament complexes, the Achilles tendon, the peroneal tendons, the tibialis posterior tendon, and a dome of articular cartilage on the talus.

X-rays show bone. MRI shows everything else. If pain or swelling has lasted more than a few weeks, or if a sprain has not settled the way you would expect, MRI is normally the test that gives a clear answer.

What a ankle MRI shows

  • Lateral ligaments (ATFL, CFL, PTFL)
  • Deltoid (medial) ligament complex
  • The Achilles tendon along its full length
  • Peroneal tendons and tibialis posterior tendon
  • Articular cartilage on the talar dome
  • Bone marrow, including stress reactions and occult fractures
  • Plantar fascia
  • Sinus tarsi and any joint effusion

Conditions diagnosed

  • Ligament sprains and tears — partial and complete ATFL, CFL and deltoid injuries
  • Stress fractures — distal tibia, fibula, navicular and metatarsal stress injury
  • Achilles tendinopathy and rupture — including partial tears and post-rupture follow-up
  • Cartilage damage and OCD lesions — osteochondral defects on the talar dome
  • Peroneal tendon tears, dislocation and tendinopathy
  • Tibialis posterior dysfunction
  • Plantar fasciitis — where soft-tissue imaging is clinically indicated
  • Sinus tarsi syndrome and chronic ankle instability

Every scan is reported by a consultant musculoskeletal radiologist through TMC, the same reporting partner used by more than 50 NHS hospitals.

Your scan, step by step

  1. Arrival. Easy parking at Strive Health Club in St Peter.
  2. Safety screening. A short questionnaire covers metal implants, pacemakers, pregnancy and previous surgery.
  3. Positioning. You sit in our reclining MRI chair with your ankle inside the open C-shaped scanner. The rest of you stays outside the magnet.
  4. Scanning. A typical ankle MRI takes around 25–35 minutes. Ear protection provided.
  5. Aftercare. None. You can walk or drive straight out.
  6. Report. Your images and a full TMC consultant report are normally delivered within 3–5 working days.

Ankle MRI Jersey — frequently asked questions

My sprain was three months ago and still hurts. Is MRI worth it?

Probably yes. Persistent pain after a sprain is exactly the situation where MRI is most useful, because it shows ligament tears, cartilage damage and occult fractures that X-rays will miss.

Can you scan my Achilles tendon?

Yes. The Achilles is well visualised on a standard ankle MRI protocol. Tell us at booking if Achilles imaging is the main reason for the scan — we will make sure the protocol covers the full tendon.

Do I need to be in a cast or boot when I arrive?

If you are currently in a removable boot, you can take it off for the scan. Plaster casts may need to be removed by your treating clinician before MRI — please ask us before booking.

Will the scan show why my foot still swells after running?

An ankle MRI will identify most soft-tissue and bone causes of persistent post-exercise swelling — tendinopathy, stress reaction, ligament injury and joint inflammation.

Is the scan safe with metalwork from previous ankle surgery?

Most modern orthopaedic metalwork is MRI-conditional and safe to scan. Send us a copy of the implant card if you have one, and we will confirm before your appointment.
